Registered Nurse Case Manager
DESCRIPTION OF POSITION/RESPONSIBILITIES AND DUTIES:
- Responsible for Case Management of assigned patient.
EDUCATION AND WORK EXPERIENCE
- Graduate of an approved school of Nursing.
- Minimum of one year clinical experience in acute medical/surgical care (hospital, SNF, or LTAC) required. Home Health experience preferred.
- Basic computer skills.
- Current Louisiana RN license; CPR certification; Possess and maintain valid drivers license; Maintain required hours of continuing education.
- Ability to read, write and speak intelligible English; Ability to communicate with patient, family, peers and medical personnel.
- Requires emotional stability and resilience. Must be adaptable to change, be decisive and demonstrate foresight. Exhibits organizational and leadership ability.
Physical Demands/Work Environment
- Work requires a variety of physical activities including sitting, standing, walking, bending, stooping, lifting and ability to get in and out of vehicle.
- The work environment may be physically confining.
- Must have normal hearing.
- Visual acuity is required.
- Must be able to drive a vehicle with good driving skills in both daytime and nighttime.
- Must be able to remain in a stationary position for long periods of time.
- Good manual dexterity required for keyboard activities.
- Ability to work weekends, overtime, and on-call required.
- Must be free of all communicable disease and be up to date on immunizations.
